Following the release of Report of the Armoured Cavalry Programme (Ajax) Lessons Learned Review [1], SMP02 [2] has been updated to recognise Recommendation 18 – “Information to senior personnel at DE&S [1*-4*] and the SRO”.

Recommendation 18 recommends that any dissenting/contrarian opinions from personnel within the project safety committee (PSC) should be adequately considered and recorded.
