Audit Question Set Section 5.4 Consultation

These questions can be used as an aid to assure that your project adheres to the policy briefed in clause 5.4 of the Statute Policy mandated by Defence Regulators which states;

Operating Centres, Delivery Teams or equivalent shall ensure that all stakeholders are identified and consulted so that their views and responsibilities are considered when managing safety risks and the delivery of sound environmental performance.

Organisational Leadership, Culture, Capability and Change Management
  1. How did management identify its key stakeholders and determine those issues upon which it needed to consult with them?
  2. Where are details of stakeholders, stakeholder agreements and the results of consultation held?
Risk Assessments and Safety Cases
  1. How did the team assess which stakeholders needed to be consulted on risk management issues and where are the details recorded?
  2. What roles did stakeholders play in risk assessment activities and where are the results of their participation recorded?
Self-Assurance
  1. How has the team assured itself that its stakeholder management processes are adequate and effective and where are the results recorded?

GMP02

Communication

Effective communication of the principles, goals and outputs of the Delivery Team’s Safety Management System and Environmental Management System is essential to the success of managing safety and environmental risks. It is important to ensure that relevant responsibilities are understood and that actions should be carried out in a coordinated and efficient way.

EMP02

Stakeholder Communications

This procedure will enable the Delivery Team to ensure that environmental issues are appropriately managed, with all environmental responsibilities communicated. EMP02 will assist the Delivery Team in identifying stakeholders, define and agree project environmental responsibilities, produce an effective communication plan and assemble an environmental committee.

SMP01

Project Safety Initiation

This procedure describes the start-up of safety management activities on a project. It identifies safety stakeholders, and legislative and other standards that should be satisfied. The procedure also creates the key elements of the safety management organisation for the project.

SMP02

Safety Committee

This procedure describes the implementation of a Safety Committee, in order to ensure effective co-ordination between persons holding safety responsibilities.

SMP04

Preliminary Hazard Identification and Analysis

This procedure provides guidance for conducting a Preliminary Hazard Identification and Analysis in order to determine the scope of the safety requirements for the system.

SMP05

Hazard Identification and Analysis

This procedure provides guidance through the Hazard Identification and Analysis process, in which all credible Hazards are identified and subsequently analysed in order to establish the associated Accidents and Accident Sequences.

SMP07

Risk and ALARP Evaluation

This procedure provides guidance through Risk and ALARP Evaluation in order to identify where the system Risk meets Tolerability criteria, and where further action is required.
